The Philadelphia Zoo appreciates you taking time to complete our Supplier Information Form. Following this introduction letter you will find a series of general information questions that will request the following:
  • Supplier Contact Information
  • Remittance Address
  • Organizational Structure
  • Minority Classifications
  • Email Address
  • Federal Tax ID#
We ask that you complete this form in order to ensure that future transactions between the Philadelphia Zoo and your company are timely and accurate. Please note that if for any reason any information for your organization changes, we request you to access the link titled “Supplier Information Form” and update the contact information. Failure to do so may result in improper transmission of purchase orders and related payments.

As of August 30, 2010, the Philadelphia Zoo converted to electronic Purchase Orders. All Philadelphia Zoo Purchase Orders will be transmitted via e-mail. By moving to electronic commerce, the Philadelphia Zoo is demonstrating its ongoing commitment to reducing our carbon footprint and becoming more streamlined with our mission statement. In order to assist us in this transition and ensure that we are able to transmit Purchase Orders to your organization, we ask that you fully complete the Supplier Information Form.

We ask that you direct any questions regarding the Supplier Information Form or the Zoo’s transition to Electronic Purchase Orders to the Purchasing Department at purchasing@phillyzoo.org.
